Required and Necessary Forms

The summer tours are a memorable part of the youth exchange experience – and often the highlight. You will find the forms that are required to make a reservation on this page. The forms are listed in an interactive form to make it convenient for you to fill out BEFORE you print them for signatures.

All students making a reservation must complete:

1) WINTER RESERVATION FORM (available below – PLEASE NOTE that the PDF Adobe Acrobat reservation form is interactive which means you should fill in the requested information BEFORE you print it. This will eliminate misreading the information that you provide.)

2) SUMMER RESERVATION FORM (available below – PLEASE NOTE that the PDF Adobe Acrobat reservation form is interactive which means you should fill in the requested information BEFORE you print it. This will eliminate misreading the information that you provide.)

3) PARENTAL CONSENT FORM (available below - Parents of student may download the consent form in their home country to save time in mailing or faxing the document to them.)
